Athletics Forms

Proof of a physical exam must be signed by a physician and submitted to the Athletic Office (No telehealth physicals accepted). Physicals are valid for 365 days.

All Athletic Physicals MUST be emailed to jgregory@svak12.org. We cannot accept paper copies at this time.

ALL middle and high school athletes MUST have a sports physical dated June, July, or August to participate in any sport at SVA.
